UDLCO project : Evidence for lowering incidental hypertension in people with no initial metabolic vascular risk factors

Summary : Following an introduction to a lecture delivered on hypertension management by an international speaker, a single paper was singled out from his slides and the evidence for lowering incidental hypertension in people with no initial metabolic vascular risk factors debated.

UDLCO Conversational transcript :

[11/5, 5:15 PM] Cardiovascular community Moderator : 3 days of CAD conference in Mumbai.

Many of our mentors of ... were the faculties . Also we had excellent international faculties from UK , USA & European countries ( Like Dr Bernard Gersh , Brian Williams to mention few names + others )

We will try to send key take away points ( In fact few I have already shared earlier , some more I shall share in course of time )

Few of our presentation ppt are already shared . ( Thanks for sharing today morn. One of the best in the subject , Friends pl go thro every slide)

Congratulations Dr... & His entire team of course directors for organizing the conference , with superb academic contents .

Please block dates Feb 9 , 10 & 11 , 2024 for CAD INTERVENTIONS Conference . The further details will follow !

[11/5, 5:40 PM] Cardiovascular community Moderator
: BP PRESSURE TARGETS : WHETHER TARGETS SHOULD BE LOWERED ?

Dr Brian Williams ( OPPONENT )

FRIENDS , Please go thro this presentation , one of his best presentation he has delivered ! ( During so many of his talks in CAD conferences) ! ( Please Go thro inch by inch )

A string of slides are shared numbered 1-16 by Cardiovascular community Moderator

[11/5, 8:29 PM] Rakesh Biswas: 👆Of the 5684 events mentioned here in slide 16, it would be important to know how many had no comorbidities (as in slide 11) and had nothing other than hypertension.

[11/5, 8:51 PM] Rakesh Biswas: Was intrigued enough to browse the entire paper here 👇

Appears to make a lot of assumptions based on selection bias perhaps!

The people with higher titre could be the one's who had a higher likelihood of caring for other features of their health and not just BP?

[11/5, 8:55 PM] Cardiovascular community Moderator

: People were excluded !

(i) if they had prior history of hypertension before study inclusion
(hypertension diagnosis or prescription of BP-lowering medication)

(ii) If they had prior history of any of the cardiovascular diseases used as study endpoints

(iii) Follow-up of less than six months since the third BP-measure.

Individuals eligible for study inclusion were eighteen years or older, and registered with their practices for at least one year.

[11/5, 9:04 PM] Rakesh Biswas: Unlikely that 5000 people would die of cardiovascular events within a few months of being diagnosed as hypertension without having any cardiovascular risks? Was that missed? The study is just based on retrospective EMR analysis and I wouldn't be surprised if it was missed

11/5, 9:06 PM] : Concept & key take away points Rakesh !

■ Based on the current frequency of measurement of blood pressure this study suggests that few newly hypertensive patients sustained a complete, year round on target BP over time.

■ The inverse associations between a higher TITRE and lower risk of incident cardiovascular diseases were independent of widely used blood pressure
' control’ indicators.

■ Randomized trials are required to evaluate interventions to increase a person’s time spent at blood pressure target.

[11/5, 9:05 PM] Rakesh Biswas: I'm becoming a fan of this presenter!! 👏

[11/5, 9:15 PM] Cardiovascular community Moderator
: Agree with u Rakesh , I have heard him so many times in CAD Conferences , but this one talk was simply amazing one .

In fact he likes CAD conferences so much ( He says so many people in hall till 8.30 PM etc )

Plus we could interact with him thro his mail !

Outstanding Orator & Practicality in talk ( Truely we have to admire Dr mardikar for getting him , Dr Gersh , in particular along with other international faculty) !

Next question on Slide 15 that shows a question about the quality of BP control:

" What percent of time in an individual is the BP controlled!"

11/5, 9:07 PM] Rakesh Biswas: Is there any study that has addressed this question other than the next slide projected study?

[11/5, 9:17 PM] Cardiovascular community Moderator
: The next slide only ( slide 16 , Preceding is the title )

[11/5, 9:33 PM] Rakesh Biswas: Yes amazing that this hasn't been looked at earlier and we need a better robust study than an EMR review titre to do justice to this question!
